How to Write a BIRP Note (With Examples)

The BIRP note, Behavior, Intervention, Response and Plan, is popular in community mental health, case management and any setting that must clearly demonstrate medical necessity, because it foregrounds what you did and how the client responded.

1. Behavior

The client's presentation and reported symptoms this session, both what they say and what you observe.

Example: "Client presented tearful and reported increased panic episodes at work, describing racing thoughts and a sense of dread most mornings."

2. Intervention

What you did in session and the clinical rationale. This is the section payers scrutinize, so be specific.

Example: "Delivered psychoeducation on the fight-or-flight response and guided the client through a paced-breathing exercise; used cognitive restructuring to examine catastrophic predictions about work."

3. Response

How the client responded to that intervention, engagement, insight and any in-session change.

Example: "Client engaged readily, reported a drop in subjective anxiety from 8/10 to 4/10 after the breathing exercise, and identified one alternative thought independently."

4. Plan

Next steps: homework, interventions to continue, referrals and the next appointment.

Example: "Client to practice paced breathing twice daily and keep a thought record. Continue weekly sessions; revisit workplace triggers next week."
